When you register as a patient of the surgery you may wish to name a preferred GP for your medical care. You are welcome to change your preference at any time-please inform the Receptionist if you wish to do so.
If you do not have a NHS Card to save you time when you come to the surgery to register you may wish to print off the registrAtion form below and bring it with you. One form is needed for everyone who wishes to register.
When you register you will also be asked to fill out a medical questionnaire.
This is because it can take a considerable time for us to receive your medical records. There is an online version of this form, which you may print off, fill out and bring with you when you come to register.
Both of these forms need to be printed, completed and then brought, or posted, to the surgery. they cannot be submitted on-line.
If you take regular medication please make an appointment to meet with our primary care pharmacist to enable them to discuss your health needs and review your medication. Once this has been done you are able to order your repeat medication either on-line or via our prescription telephone line 01643 703441 (Option 2).

If your correspondence relates to a request for medical treatment or any urgent matter please contact our Patient and Practitioner Services team on 01643 703441 (8:30am – 6:30pm). Our prescription line is available for any medication queries or prescription requests on the same number (9am - 12 noon, Monday – Friday).
Our GPs and clinical team cannot provide medical advice by email. General information for the attention of a GP will be passed on if requested.

This email is for repeat prescription requests only.
If your medication request is urgent please contact our prescription line on 01643 703441 (Option 2). 9am - 12 noon, Monday - Friday.
Please include your name and date of birth with your request. Please be advised that requests for “all repeats” or requests with limited information are likely to result in a delay in the processing of medication. In such instances, staff will need to contact you to discuss your exact requirements.
